Why it matters
OCD is treatable. But first-line treatment doesn't work for everyone.

Most OCD starts with ERP therapy and medication. Some needs more.
TMS may be a fit if you
- — Are an adult with OCD diagnosed by a clinician
- — Have tried two or more medications without enough relief
- — Have tried ERP therapy and still struggle with obsessions or compulsions
- — Couldn't tolerate the higher medication doses OCD often needs
- — Can come in for daily sessions over about 6 weeks
- — Have no metal implants in your head and no history of seizures
Start with a standard OCD consultation if you
- — Haven't tried ERP therapy or an SSRI yet
- — Are newly diagnosed or looking for a first evaluation
- — Aren't sure if it's OCD, anxiety or something else
- — Have OCD symptoms during pregnancy or after having a baby
- — Mainly struggle with depression rather than OCD
- — Are in crisis right now (call or text 988)
What to expect
What your first OCD visit actually looks like.
No surprises. Here's how it works, from your first call to your treatment plan.
We learn your history
A clinician calls within 24 business hours. Have a list of every medication you've tried for OCD, the dose and how long you took it, plus any ERP therapy you've done.
A full psychiatric evaluation
We review your obsessions and compulsions and how much of your day they take, and screen for conditions that often come with OCD, such as depression, anxiety or tics.
Measured, not guessed.
You complete the Y-BOCS, the standard OCD severity scale, so we can track your progress at every visit.
You leave with a clear next step
That may be starting or adjusting medication, an ERP therapy referral, or, if you qualify, TMS therapy.

Common questions
OCD FAQs.
OCD involves unwanted, intrusive thoughts (obsessions) and repeated behaviors or mental rituals (compulsions) you feel driven to do to ease the distress. They take up a lot of time, often more than an hour a day, and get in the way of daily life. Everyday worry doesn't usually come with rituals.
For most people, it's exposure and response prevention (ERP) therapy, an SSRI medication, or both. These first-line treatments help many patients. TMS is an option when they haven't worked well enough.
OCD medications usually take 8 to 12 weeks at an effective dose to show their full effect, and OCD often needs higher doses than depression. That's why it's important not to give up on a medication too early.
Don't stop it on your own. Talk to your prescriber about the dose, the timing or a switch, and whether you've had enough ERP therapy. If two or more medications haven't helped enough, you may qualify for TMS.
Yes. Specific TMS systems are FDA-cleared for adults with OCD, used alongside standard treatment. It's not a first treatment. It's for people whose OCD hasn't responded well enough to medication and therapy.
No. SPRAVATO is approved for depression, not OCD. If you have OCD along with depression that hasn't responded to antidepressants, SPRAVATO may be an option for the depression.
Yes. ERP works best when it continues alongside psychiatric care. We manage the medication and TMS side, and with your permission we coordinate with your therapist. If you don't have one, we can help you find an ERP-trained therapist.
We accept most major insurance plans, and coverage is confirmed on your screening call. Coverage for TMS for OCD varies by plan and usually requires documented past treatment, which we help you gather.
Yes. OCD that starts or gets worse during pregnancy or after delivery is common and treatable. We offer perinatal psychiatric consultations, including a careful review of medication options during pregnancy and breastfeeding.
